Birthday Party Release Form

I, as the legal guardian of the designated student(s), grant my consent for my student(s) to participate in the programs offered by this facility. I recognize that activities involving height or motion, such as tumbling, cheerleading, tumble track, trampoline, stunting, pyramids, dance, martial arts, gymnastics, and general physical activity, carry a risk of potentially severe injuries. I acknowledge that the staff and personnel are committed to ensuring the safety and well-being of my student(s). In exchange for permitting my student(s) to utilize these facilities, I hereby COVENANT NOT To SUE and to FOREVER RELEASE this facility, its affiliated and partner companies and organizations, property owners and lessors, staff, contractors, subcontractors, teachers, coaches, owners, directors, and other involved individuals associated with this facility's programs, from any liability for all damages and injuries sustained by my student(s) during lessons, supervision, and/or control in any classes, practices, camps/special events, or additional activities.


I hereby grant authorization to Cheer Athletics, Inc. and its representatives to provide medical treatment for myself or my child in situations where I am unavailable to give consent. Furthermore, I consent to Cheer Athletics, Inc. and its representatives administering necessary emergency care to stabilize and/or improve the existing injury or condition that my child or I might sustain during activities associated with Cheer Athletics instruction, practices, or performances. This consent does not require a prior determination of a life-threatening emergency or the danger of serious permanent injury resulting from the treatment. Cheer Athletics strongly advises that all athletes/participants maintain their own health insurance coverage to help defray the expenses related to any injury or illness. The instances exempt from this authorization are as follows:

PHOTO/VIDEO RELEASE: I authorize Cheer Athletics, Inc./CA Brands, LLC to use photographs, video, and/or other likenesses of myself or my child(ren) for use in its promotional materials or sales and waive any rights of compensation or ownership thereto.

You retain the option to request a physical paper copy instead. However, by selecting this checkbox, you are relinquishing this preference. Following your consent, you can, by submitting a written request to us, acquire a paper copy of an electronic record. There will be no charge for obtaining such a copy, and no specialized hardware or software is needed to view it. Your agreement to employ an electronic signature for any documents with us will persist until you formally inform us in writing that you no longer wish to employ an electronic signature. Withdrawal of your consent carries no penalties. It is essential to ensure that we have your current email address at all times so that we can reach you regarding any necessary updates or changes. By checking here, you are consenting to the use of your electronic signature in lieu of an original signature on paper.

I am either the Participant (18 years or older) or the parent or legal guardian of the Participant. I have read the above "CA Plano, LLC Medical and Liability Waiver: Statement of Medical Condition, Assumption of Risk, Release, and Indemnification," "Medical Consent" and "Use of Pictures(s)/Film/Likeness" policies. I understand, agree, and consent to all terms therein on both my behalf and, if applicable, on behalf of my minor participant.
